Daniel Polonsky, Psy.D.

Dr. Daniel Polonsky is the founder and owner of Cognivia Assessment and Therapy, PLLC. As a licensed clinical psychologist and neuropsychologist, he provides intake evaluations, comprehensive neuropsychological and psychological assessments, psychotherapy, and patient feedback. Dr. Polonsky is dedicated to offering excellent and compassionate care, believing that successful treatment begins with active listening and collaboration, integrating medicine and psychology, and working closely with families and the community.

Dr. Polonsky works with individuals across the lifespan with neurodevelopmental disorders, degenerative diseases, complex neurological conditions (e.g., epilepsy, cerebral palsy, and hydrocephalus etc.), sleep disorders, and affective conditions. He has extensive experience in outpatient clinics, hospitals, and nursing homes, and has trained and supervised doctoral-level psychology students.

As an adjunct professor at The Chicago School of Professional Psychology (TCSPP), Dr. Polonsky teaches courses on the biological and neurological foundations of psychiatric disorders, complex neurological diseases, and understanding the mind-body connection. He also guest lectures to academic and clinical audiences.

Dr. Polonsky earned his B.S. in Brain and Behavioral Sciences from Purdue University and his Master of Arts in Clinical Psychology and Doctor of Psychology in Clinical Psychology with an emphasis in Clinical Neuropsychology from Adler University. He completed a two-year fellowship in neuropsychology at ARCC Neuropsychology. His research includes work in memory, epilepsy, and neuroimmunology.

Fluent in English and Russian, Dr. Polonsky is committed to helping patients navigate their cognitive and emotional well-being with empathy, insight, and scientific precision. Outside of his practice, he enjoys golf, music, reading, traveling, and exploring new experiences.
