Do the Neuropsychologists and Psychologists prescribe medication?
Our clinicians do not prescribe medications. In coordination with the patient’s primary care physician, neurologist, and/or psychiatrist, our doctors may make pharmacological recommendations.
How can I obtain a copy of my records?
To obtain your personal records, please contact our office and we will forward them to you or arrange them for pick up as requested. Please note that photo identification will be required for in-person pick up of records.
I am the Healthcare Power of Attorney (POA) for the patient. What do I need in order to obtain a copy of the evaluation report and/or records?
The office will request a copy of the Healthcare Power of Attorney document to be forwarded to us prior to being able to discuss records requests for the patient. Please note: records will only be released to the Initial POA as listed on the document. Records will only be released to the Successor POA when documentation of the resignation of the primary Agent is provided.
How do I have my records forwarded to another clinician?
To have records forwarded to another clinician, please contact the office at (847)-449-6084 and you will be provided a Release of Information Form to be completed. Upon completion and review, all requested records will be forwarded. There is no fee for continuation of care.
